Our Office and Policies

Appointments

Appointments can be made by calling our office during normal business hours. If you are a New Patient to our practice or have not been here within the past year you will be asked to do new patient paperwork. If you are more than 15 minutes late to your appointment, we will be glad to see you, but on a work-in basis as the schedule allows. You may also choose to reschedule to next available appointment time. If you No Show for your appointment, you will be required to pay a $25 No Show fee in order to schedule another appointment. Due to the number of patients that we see, please limit the number of family members that you bring to your appointment to 1 or 2. Please be sure to bring your drivers license (or some other picture ID) and your insurance cards to your appointment.

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, we do have an on-call physician who is available 24-hours a day, 7-days a week.

Prescription Refills

Prescriptions are refilled during normal office hours. If you have not seen your doctor within the past year, we may require that the patient be seen in our office prior to a prescription is renewed. We use electronic prescription refills, so please request that your pharmacy request your refill electronically to us.

Financial

All co-pays and deductibles are due at the time of service. We accept cash, checks, most major credit cards, debit cards and Care Credit. Please contact our billing office should you have any financial questions or concerns. We also offer Online payment option for our patients to pay their balances. We offer Care Credit as an option for Hearing Aid financing.

Out Patient Surgeries

If your doctor feels that it is medically necessary for you to have a surgical procedure performed, you will meet with our Surgery Coordinator to discuss your procedure, insurance coverage and to schedule your surgery before you leave the office. If you No Show for your surgery, you will be required to pay a $200 No Show fee in order to reschedule your surgery.
